CareCoordinator for Post Acute Care is our original version. It is focused on Resident Resource Coordination and Transportation Logistics.
The system is Transport and/or Resident/Patient centered with each activity scheduled as an appointment with all of the required resources attached. Managing transporation becomes much easier when each movement is associated with all of the required resources.
Resident appointments, both internally and externally are more easily managed because of the resource coordination built in.
CareCoordinator for Home Care is our newest version. It is focused on Caregiver Scheduling, Care Notes, & Client Care Plans.
With the systems ability to present different portals to each of the different roles, Caregivers, Clients, Administrators and Office workers get to the information they need quickly.
Secure Communications is always a challenge with Caregivers using personal devices. The systems built-in Communications allows messaging without the use of email or texts. They never leave the system and remain secure within the application.
Care Plans with integration into Session Activity Tracking makes performing the required assists for each Client clear. Session notes and Session Timers assist Caregivers in carrying out their duties while providing the Agency with much needed information to better plan and to provide better care.
